Great doctor and was recommended highly by my relative. He diagnosed well and my feet are fungus free.

Dr. A is a surgeon and that seems to be the meat of his practice (as opposed to actual, periodic patient care). His billing preactices are dubious (my word), based upon making financial offers which he then doesn't adhere to as well as charging both you and your insurance company for the same service. His "insurance company rates" are at the upper limit. He wants you to buy over-the-counter medications from him for your maladies.
