I am a Board Certified Family doctor who has practiced in the Denver area for twenty five years. My practice, Summit Family Medicine, is part of MDVIP, a national network which comprises 600 highly regarded physicians across the United States who specialize in personalized care, wellness and prevention. Independent 2011 data from the 190,000 MDVIP patients shows that Medicare patients are hospitalized at an 80% lower rate than the general Medicare population and the commercial insurance population is hospitalized at a 65% lower rate.
I limit my practice to a small number of patients so I can provide same or next day appointments which last a minimum of thirty minutes. We strive to be on time and generally there is no waiting for the patient. The patients who choose to join my practice begin their year with an annual 2.5 hour Health Assessment that is the cornerstone of our prevention and wellness program. The cost is $1500 and is not billed to insurance. The fee paid by the patient covers the testing and all of the comprehensive lab work associated with this wellness program. We conduct an extensive risk factor assessment that incorporates family history, patient history, lifestyle analysis as well as screenings related to mental status, exercise, nutrition and sleep. We do onsite EKG, pulmonary function, vision and hearing screening. After all the blood work and results are gathered, I meet with the patient and do an extensive exam and we discuss the wellness action plan. This may include coordinating specialty care, home health care, meeting with family members or anything that will facilitate improved health for the patient.
We strive for same day appointments and I am also available to my patients 24/7 via my cell phone. Because I have a close relationship and know my patients well, I can often take care of their problems immediately and save them a trip to Urgent Care or the ER. Please see my website, JonCramMD.com.
